I've done Rome, Orvieto, Fiesole and Florence, but not Milano. If you have any local contacts you trust at all, get restaurant recommendations from them. Don't go to restaurants in the touristy areas, and for god's sake don't go into restaurants that are full of Brits.
Getting food in the daytime, sandwiches to go: Know that 'panino' is singular and 'panini' is plural. Look for the little 'Tavolo Calda' (hot table) places. Find the ones that are jam-packed full of locals at lunchtime. That's where you want to buy a panino or a pizza.
And yeah, post a photo of the right city next time.
