You should definitely go to the Vatican. Probably one of the most impressive structures I have ever seen period. Go with a tour group, unless you really just like waiting for 4 hours in a line to get into the art museum. Climb the Basilica and visit the catacombs. OP's pick looks like it was taken from the top of the Basilica. In fact, in any city you go to with a cathedral or a tower, climb those stairs and go to the top. It has some of the best views of whatever city you are in.

One thing thats great when you are tired are these double decker red buses that drive you around to different sites. You buy a pass and then you can hop on and off at any of the stops and it saves you a bit of walking. I did it in Florence one day and it was pretty cool. Not sure if Rome has it, but surely they do.

The Colosseum and the Forum are must sees. Located nearby is the Palatine Hill where you can explore the old palace. Your ticket into the Colosseum also covers Palatine Hill and the Forum is free.

Go and see The Monument of Vittorio Emanuele II, there is a free museum located inside that is really cool. Big statues, historic military uniforms and weapons, etc etc.

Rome was good fun. The roads, however, are a major health hazard. Don't look at the lights, just wait till there's no traffic about and RUN. The drivers there stop for nothing.

Rome is 1285 square kilometers of fucking win. Like the coliseum for example: You see it in tacky postcards and cheap souvenirs and don't think much of it. But then when you see it, and smell that weird smell that never goes away (which everyone says is all the blood that seeped into the ground over hundreds of years) and see several centuries' graffiti carved into the wall, you realize that all that shit REALLY HAPPENED, it permanently changes the way you see the world. And then you look around you at all the stupid tourists making stupid jokes and you wonder how they'd react if they had to sit in a stadium and watch criminals get eaten by lions.

Spend an evening hanging out in Trastevere. It's more like a real Italian neighborhood, with not so many big, dangerous intersections. People actually stop at the stoplights. Also the Americans and Brits you run into in Trastevere tend to be younger and better-looking than, say, the tourists who stay in the Centro Storico.

The Centrale Montemartini museum. Formerly an electricity-generating plant, it was temporarily used as a warehouse for statues while some other museum was getting remodeled. Then some genius figured out that they actually looked pretty cool against the turbines and guages (this was in '97, before 'steampunk'. So they turned it into a museum.
