You should definitely go to the Vatican. Probably one of the most impressive structures I have ever seen period. Go with a tour group, unless you really just like waiting for 4 hours in a line to get into the art museum. Climb the Basilica and visit the catacombs. OP's pick looks like it was taken from the top of the Basilica. In fact, in any city you go to with a cathedral or a tower, climb those stairs and go to the top. It has some of the best views of whatever city you are in.
One thing thats great when you are tired are these double decker red buses that drive you around to different sites. You buy a pass and then you can hop on and off at any of the stops and it saves you a bit of walking. I did it in Florence one day and it was pretty cool. Not sure if Rome has it, but surely they do.
The Colosseum and the Forum are must sees. Located nearby is the Palatine Hill where you can explore the old palace. Your ticket into the Colosseum also covers Palatine Hill and the Forum is free.
Go and see The Monument of Vittorio Emanuele II, there is a free museum located inside that is really cool. Big statues, historic military uniforms and weapons, etc etc.
