Depends if you're a nature guy, temple guy, and so on. It's easy to get sick of one type.
First, I'd say browse japan-guide.com to get an idea of what interests you. It leans more towards touristy activities, but that may be exactly what you want.
My standard recommendations are Tokyo, Kyoto, and Hiroshima. They're well spaced out so you can see most of the country this way. From each main city, you can break it down into individual things to see or do. Again, check the website.
To give you some recommendations you won't typically find online: Kichijouji is a cool area near Tokyo that young adults hang out at. It's the largest non-trashy park you can find with lots of people sitting around drinking, enjoying the performers, and there are vendors set up as well. If you're weeaboo, there's a Ghibli museum there.
Since you mentioned museums first, there's one in Yokohama that's so-so; there's a much more impressive one named MOA that is built into the side of a mountain in Atami - about an hour away. If you head towards Atami, you can explore the Izu peninsula's coastal scenery and maybe do some outdoor onsen bathing. Shimoda is another relaxed area with many monuments dedicated to U.S.-Japan relations.
Clubs... Roppongi is seedy, but it's important to see that side of Japan as well. Odaiba has some nice venues by the water. If you're into a genre, search online for some groups and find a live show in Tokyo somewhere. You can meet some good friends this way.
The mecca of "unmissable places.. weeaboo and cultural" is Akihabara. Go there.
