Why does 90% of the league have bullpen problems?

Are good relievers really that hard to come by?

Is it because most of the GOOD relievers end up getting put in the rotation?

exactly. good pitchers either become:

A) starters
B) closers
C) 8th inning set-up guys

the middle relievers are usually reserved for old pitchers who never made it and prospects who may one day make it into one of the above categories.

Teams don't focus on their relief situations until its too late

Also, Relief guys are pitching multiple innings everyday, where-as Closers pitch one inning a day and starters pitch whatever innings every 5-6 days. They burn out fast because they don't have the endurance Starters do and they always have to get the Starter out of trouble and set up the Closer.

Jays bullpen never seems to have much of a problem (Except Jason Frasor and BJ Ryan this year).

The ideal situation for your middle relief is to pitch multiple innings in one outing and then rest for a game or two. The Dodgers currently have this where they can throw Chan Ho Park, Kuo, Cory Wade, and Ramon Transcoso out on any given day. The problem is that most teams only have two quality guy, their closer, and their 8th inning guy, so they overuse and abuse their best pitchers, and eventually blowing them out.
