I think it's technically possible, but it's the sort of situation that is almost impossible to see happen accidentally. If a team manages a safety on a two point or PAT attempt, the other team gets a point.

In the NFL, the team that scored a TD can get a 1-point safety if the defense manages to recover a fumble on a 2-point conversion and then runs it into their end zone, where they are tackled. Blocked PATs are dead balls.

In college, blocked PATs are not dead, and thus it's a bit easier. Also, the defense can get a one-point safety in the highly unlikely instance of, say, the snap to the PAT holder going 103 yards down the field and out of the end zone. (Or if the defense recovers a fumble/blocked kick and runs it back, only to then fumble themselves, have it recovered by the offense, who then runs the ball backward into their own end zone.)
