Did it look something like this? It's from a band called "The Residents". I remember seeing someone post pics of a home-made version, but I don't have those.
A .pdo was posted, though I'm not sure if it was his, because the one in the .pdo would come up to be palm-sized. You could still scale it up, though. Let me see if I can find.

Um... you open a .pdo in Pepakura Viewer, then pick File / Copy Development To Clipboard, then paste it into your art program.
