you need the ffxi model viewer http://www.noncouncil.com/phabin/ffxi/?i=mv

and Metasequoia http://www.metaseq.net/english/index.html

Use the viewer and choose your model and then export to a .mqo file. Wich you can open with meta, then export from meta to a .3ds file to open with prepakura

You can open *.mqo files with pepakura; many people report having problems using *.3ds files with metasequoia and pepakura, so instead of converting the *.mqo to *.3ds, just keep the *.mqo. It will save you a step and maybe a lot of problems.

These are the formats that pepakura supports: http://www.tamasoft.co.jp/pepakura-en/productinfo/func5.html

The small print says: "However, these formats may not load properly depending on output applications and versions."

Metasequoia has never ever given me any problems (those two seem to be made for each other; must just be a coincidence that they're both originally Japanese ;o). So if you have it, I'd use that instead of anything else; it'll save you a lot of annoyance.
