As for the feet, there's no formula or scale table for that but I just guessed and tested different scales by printing on a scrap piece of paper and putting my foot on it. The bottom of the boot (the one that looks like a footprint drrr) attaches to a toe piece; so when you have your foot on the printed piece, your big toe shouldn't be within the line.
That leaves the chest piece, forearms and calves which nobody have come up with anything for. The chest piece is the same scale as the helmet for a lot of people but I can't confirm how well this works; mine worked out a little differently due to boobs and only one guy has posted his scale and measurements.
