well I've made kind of a template, it's hand-drawn and not very symetric but that's the way I made it out of cardboard

just copy the template on some cardboard.
To adjust the proportions measure the width of your own face as basis;
once you've cut and folded the top part, try it out to adjust the holes for your eyes

it's a guesstimate indeed...as was the original.
but card board is great in that you can always rearrange when you mess up; the whole thing was made in trial and error: the point is not to get an exact replica but a mask that's customized to your own proportions

about the moving jaw I guess if you use paper fasteners to hold it, and have your chin tightly held by the band, it may be possible...
