Make a cylinder with a good number of slices and steps, then cut off the back half of it. Stretch the vertices to points on the face you think will give you an accurate shape.
Once you have all the vertices at the points you want, flatten them along the Z axis, then stretch them along that axis to add the depth where needed to make the nose, lips, cheek bones, brow, and hair line.
Apply the photo as a texture on the model.
Export from milkshape as a .3ds
Import into pepakura and finish the process (plenty of video tutorials for pepakura are available on youtube)
