self esteem :) and yes, staring at yourself in the mirror. look at past pictures of yourself and take new ones.
if a professional photog is taking your portrait, he should have some clue on making you relax. i talk a lot to my subjects before shooting and keep the camera nearby so they can get used to its presence. sometimes i even give them the camera for a while and let them play with it (believe me, it helps!).
on normal point-and-shoot pics, direct on-camera flash makes anyone look ugly.
personally, after taking up photography more seriously i slowly stopped worrying about how i look in other peoples' pics. i've seen countless pictures of my face and know what my expressions look like and while paying attention to the surrounding light, i can visualize the results pretty well.
