With those, it's about shifting, and the tilt is just to compensate for the angled bellows, in normal circumstances. Basically, shifting allows the light rays to come from a different angle than usual, but the focal plane is still parallel to the subject, so straight lines stay straight. It's pretty crazy in action on the big screen of a view camera, it looks just like if you picked up the camera and carried it around, except you're staying still.
When taking pictures of something tall, it looks exactly as if you're floating in the air with the camera a couple stories up, pointed directly at the subject.
