Macro is gearfag nirvana. This is just the tip of the iceberg, and it can all be put together in a million different ways. Or you can buy a 100USD digital point and shoot and do quite alot, actually.
The biggest difference in all the setups is distance to subject. Also cost, but not always. That is why the endless gearfag arguments over which setup is better is pointless.
The determining factors in the example you posted are luck and patience, in equal quantities--assuming it was alive when photographed.
