All I know about panaromas comes from here:

http://hugin.sourceforge.net/

I know that there are couple of people here that do panaromas very well. I can't seem to find a good example of one right now though.

I use PTGui for my stitching, it's free if you know where to look. ;)

use a better software such as elements or ptgui.
You need your exposure to be the same for each photos, use your exposure lock.
Try to level each photo when you take it, use a tripod.
Overlap each photo more to generate more data points for the stitching.

I'd go with that. I tried using it with the pictures I took from my cam phone, and it turned out great.

Not too often. But I recognized the lamps and the building. There's a guy there who builds these rock towers and sculptures all the time.

Trees are blacked because of exposure lock to the front, i want to make sure the center is properly exposed and compared to the sides.
Also, it was around evening time, getting dark.

Took this at the top of the mountain at Miyajima earlier this year. Still experimenting with PTGui, but it seems to be working well now.
