Direct flash is out of vogue right now, but the whole direct flash look is just another visual tool, no different than, say, blurring the background or using color. It's not the evil thing people make it out to be. Works for Weegee and Bruce Gilden, after all.
If you've got someone in near-total darkness, with a few lights in the background, open up all the way to get some nice bokeh with those lights going on, turn the flash on, and you'll get a great look that'll add a slightly wistful surreal look to the photograph. I would post an example, but the only ones I have are of friends I haven't checked with about putting their pictures on 4chan.
