Lighting: One light above the subject, pointing down. Not so steep an angle that his eyes turned to black holes, but high enough that his forehead is brighter lit than his chin.

Film: In the camera. Leaving it on the desk next to the camera just wouldn't have achieved this effect very well.

Never mind the fact that guessing the ISO of film is not easy even if you had a larger copy of the print.
