60-75 degrees forward tilt. The flash will be pointing basically vertical, but the bounce card will let enough diffused light hit the subject.
When I shoot at gigs I tend to use focal lengths between 35-100. Under 35 is almost always to wide for me. But thats due to the venues stage being raising high etc.
Depending on the lighting; if the stage is spot lit, etc, i'd probably set my camera to something along these lines, and then change the settings if nessassery
aperture: /5.6 shutter: 1/5th iso: 400 flash: 1/4th -7
Reasom for the shutter speed being so slow is to let ambient light in to make the photo more interesting.
