5.11 Tactical Pants.
That's right, I said pants. Big cargo pockets that'll hold a pro midrange zoom, heavily reinforced EVERYTHING, a retention strap on the rear that works great for tripods/monopods (It's actually meant for a big maglite or a baton), pockets in the knees for kneepads, and will stand up to any kind of torture you can throw at them. On top of everything, they look reasonably respectable, and are super-comfortable, unlike regular camos or whatever, since they're designed for cops. Big beltloops hold a wide belt that's meant to carry a radio, gun, cuffs, and whatever else a cop would have, which means it's great for lens/strobe/lightmeter pouches. Anything that's less critical and not as fragile, like batteries and anything that comes in a nice solid case, gets thrown in a small Swiss Army backpack, along with water/other drinks, a light jacket/windbreaker if I think I'll need it, and whatever other crap I'm carrying.
Gonna upgrade to a ThinkTank belt system soon, though, since it's a bit more comfortable and more secure for big lenses.
When I'm travelling around, getting on planes, and whatever, I use a Tamrac TurboCyberPro something-or-other, holds all my usual camera gear (D2 and D50, 80-200, 24-85, 12-24, 50, two strobes w/ bounces etc, meter) and my laptop and related cables, little wacom tablet, etc, without trouble.
