It's a small penalty that might not matter in the long run, but on your serve, you really don't want to be giving free points to your opponent.
Foot faults are a relatively rare occurrence. In the 15 years I've watched tennis on TV, I can only remember seeing it called twice. So I guess the umpire was just not paying attention to it. I was hoping he'd call it because I wanted to see McEnroe go ballistic for more entertainment.
