I often go out with the specific intent of taking photos, but I also carry my camera with me everywhere, just in case I see something that's particularly interesting or unusual.
For an example, the picture is something I saw while on my way home from work one day. This dork trying to turn a B-double around a corner that simply wasn't wide enough.
As you can see, the dickhead driver not only failed to notice when he hit the light pole, but pushed it so far as to tear out a panel of his tank trailer.
So, my advice is go out regularly with specific intentions in mind of things to photograph, but also be prepared to take photos of anything that might pop up unexpectedly.
