What it actually refers to is how the out of focus elements of a picture are rendered. Good Bokeh being soft diffused points of light, and bad bokeh being Donut shaped circles (which look harsh in a pic). In the OP's pic for example The Red lights in the background are now flat looking circles, They are neither great, nor bad, because they aren't donuts but are neither perfectly diffused. But they are a good form of it, because they give a bit of definition without stealing too much from the pic.
The quality of "Bokeh" is determined by numerous things, such as the number of blades in the aperture diaphram, and the quality of the glass. Some lenses like the 85mm f1.4, Are fantastic for it, And other, (older 50mm f1.4's with 5 blades) Are very bad.
