I'm a fairly small/thin guy (5'6", about 130 lbs, like 9-10%bf) but i do eat like a fucking bear, its my metabolism, but i don't think i'm malnourished, i've been there and all i ever do is puke, and my stomach never bothers me during workouts. As for the rest of my exercises, it happens at random times. The strangest i can think of was when was benching. In the middle of my set it just started shaking violently. Then, when i sat up, it stopped. Of course it kept happening as i attempted to bench so i stopped and moved onto something else..
