If your GF is a massage therapist, ask her to do some points in your legs, ass, and back (in that order) a bit before she tries to work on your shoulder, arm, then hands. Especially ass. Those muscles get reeeeeeally messed up without you noticing. The reason you should do it in this order is because they're all connected, and one thing is causing the next. Also, ask her to work the shoulder blade a lot. At first, its not going to feel much different, but as things relax, you'll be good.
I can't stress this enough: Get more opinions from other doctors and massage therapists. Steer clear of Docs that recommend cortazone. That shit is not good for you. I've watched my grandpa slowly deteriorate because of that stuff. There were a host of other things going on with him, but the cortazone was what really did the most damage.
Obviously, if you really want to get those shots, its your choice. I would at least, though, get a couple other opinions from other doctors if thats what your going to do. Just going to a couple doctors and asking them basic info can reveal A LOT, but most people don't do that. They just assume the one doc they go to is right, or they don't want to insult them by going to other doctors. That is one of the biggest mistakes I see.
This is all just my opinion, but I've seen and experienced massage working these things out. One problem is that your case has gone on so long, that its gonna take some time to get it to feel better, but just hang in there.
