also, overly hyped sympathetic nervous system (as a response to dysfunctional heart) would lead to joint pain (absolutely), decreased appetite (absolutely), insomnia (absolutely), and circulation issues (but not as you described) tldr: check you adrenals
overall TLDR: FULL 100% hormonal panel + clincial cardiac stress test/GXT with EKG+Ultrasound. they may not 'get' your problem at its source, but they will actually find its footprints, so at least they can treat some aspect of it
