Cut your hair, unless you have any large birthmarks on your head then shave it quite short.
Shave that mustach properly, trim your eyebrows a bit. I know that the later sounds like a girly thing to do, but you're in need of it. If you're unsure of how to do it, talk to an irl person who does..
Eat ALOT and train 3 times per week. Ask someone at the gym for help. It's pretty much just eat, eat alot and eat quality protein like egg and chicken (and buy protein shakes). Train using heavy weights, 2x8 repetitions (when you've done 8 your muscles should feel really burning) and train ALL the muscle groups (IMPORTANT!). Lastly, give it time, you will see results after 4-6 months, in a year or two you will look fit.
Loose the chain, stick with your clothes for now. But buy yourself a good pair of shoes, simple and clean. Preferably business like ones but even sneakers would do if they're tidy enough (no gadgets on them).
Check your hygiene! Brush your teeth every morning and evening without exceptions, brush your tongue aswell.Shower every or every other day. Use a deoderant every time afterwards.
Lastly, clean your room.Throw everything nasty out like old leftovers and bottles. Bathe your cat, change the kittylitter OFTEN and make sure to keep a window open, you may not be able to smell it... but others can.
