Do we accurately perceive what is our maximal effort?
When i think that i can't possibly push the weight more than i already am, does it means i have reached 100% of my current muscle capacity?

Not quite sure what you're asking here... however when you can't possibly lift any more reps then it means you've experienced muscle failure and is a good sign that you've done a sufficient amount for that set.

If you find that after a 5 minute rest you still cannot use those muscles, then your workout is complete.

just keep a book and write down your progress every workout. Raise the weight every week if you can. Sometime you are BOUND to hit your maximum.
That's how you can be sure.

If you really want to get the very most out of every single rep that you do, then be sure to lift as heavy as possible and as slow as possible.

"Putting in that little bit of extra effort" for me usually involves doing another 5 minutes HIIT or doing another set.

When you lift something, your brain controls your muscles so that you don't use ALL of the fibers in a given group. It does this because if you went at 100%, you'd actually end up tearing tendons and tearing muscle. It's an evolutionary thing; your body only goes all out if you're in a life and death situation.

But yeah, HelpFag is right. The best you can hope for is to push yourself more every workout.
