With that said:
For your head, do "double chins". Pull your head straight back. If you're not chubby enough to get a double chin, just imagine. This is pretty much where your head needs to be. Keep it there.
For your shoulders, practice keeping them down and back. Lower them as far as you can, and pull them back as far as you can. Keep them there.
You're probably fucked at the hips too, keeping them way to extended. Squeezing your glutes will push your hips into something like the proper posture.
Do the above three things all at once, and your posture will be much better. You don't need to hold them in the most extreme position you can possibly reach, but you do need to be much closer to that position than you currently are. If you back up against a wall, your heels, hips, shoulders, and head should all touch at the same time.
Once you've got your shoulders and head back where they belong and your hips rotated right, you should find it very easy to stand firmly on the ground, without rocking up onto your toes.
