Dan John has the perfect program: Every time you want to eat at McD's, you can. All you have to do is an entire can of dog food right after you do it. The whole can.

After a while you learn to hate the dog food more than you love the cheeseburgers. A modified version of this (running to Penn Station and back instead of dog food) got my buddy to stop smoking.
