could be something called gestational trophoblastic neoplasia/disease. three things under that: partial mole, complete mole, and choriocarcinoma. the first two need sperm, and the only the first one has baby tissue.
anyway, none of these things will happen every time and are pretty rare.
if mom and/or dad had weird genetics its possible to have screwed up kids nearly every time, but that would usually just result in miscarriage/spontaneous abortions. and again, this is would be super rare, and there would probably be a family history of similar stuff.
