1)Preplan your diet as this is probably the one element of staying fit that you will have the most control over.
2) Try to increase your non-exercise physical activity (NEPA). This includes just walking farther from the parking lot for your car, using stairs instead of elevators and escalators, the list can go on. This one is like a freebie, and combined with a solid diet will actually show big in the long run.
3) You can weight train effectively several times a week in the time span you've provided us. Supplement that with some cardio several times a week (whether you want to do this on the same days as weight training is up to you, we can go on for pages on the many different ways you can do this) if you wish.
4) Of course, to cohesively make everything work have a PLAN. PLAN PLAN PLAN. I cannot emphasize this enough. Then stick to said plan. No random trips to the gym, plan em out. No random binge meals or snacks, plan em out. Like any routine or breaking of habits, this of course may not be easy but its something you probably already knew and know you must utilize deep down.
5) Don't rationalize, and if you screw up here and there and just give up completely. Ate a cheat meal or couldn't work out as you planned? Make sure your next meal (or meals) isn't a series of bad choices again and make sure you go to the gym next time. Don't rationalize that since you "screwed up today, you'll start up again next Monday." That kind of mentality usually screws up a lot of people, it screwed it up for me, and occasionaly STILL does. It's human, but know that you must do something about it.
There are a lot of good tips I can give, but these will do for now and I'm sure others will give you more.
