I've been on the pill for 2 1/2 years now, been going bareback for pretty much the entire time and I've never even had a scare. I take them absolutely perfectly which is why it's been so successful. I was on Yasmin for 2 years then I switched to Yaz. Both work great.

Yes, this. There are a number of reasons that BC can fail, but a big one that lots of women miss is taking an inappropriate dosage/formulation for their bodies. These women may do everything else right (take it faithfully, watch for medication interactions, etc.), but but still get pregnant.

There are a few warning signs as to whether or not your pill works properly. The first is mid-month spotting- if you are consistently getting your period at a time that is not during the designated week, that can be a sign that you need either a stronger pill or a different formulation. If the side effects of taking the pill last more than the 2 or 3 cycles it takes to get your body fully acclimated to the hormone(s), you may need a different pill. Also, women who are overweight or larger in stature may need a stronger pill.

The main thing a girl can do is learn to listen to her body. There are ways to tell when you've ovulated (they involve, among other things, tracking your vaginal discharge). If you find that you may have ovulated even though you're on the pill, you should get the Morning After Pill if you've had unprotected sex in the past few days.

The shelf-life is comparable to that of other medications.
The price can vary a lot depending on where you live, but in general per use they cost more than a month of pills, but less than an abortion (and 1/10,000 of the price of raising a kid).

You don't really need to keep the Morning After Pill on hand if that's what you're thinking. It's over-the-counter (you have to ask a pharmacist but don't need a prescription sort of thing) in pretty much all of the West excluding the US. In the US it's usually a behind-the-pharmacy-counter OTC, but a few states require a prescription from your family doctor or a walk-in place, iirc. Also, you shouldn't keep it around for everyday BC, as though it's anything other than a next-to-last resort (last resort being abortion), since it is effective but very very unpleasant to use. And you have around 48 hours from the last sexual episode or ovulation to use it, so you should have plenty of time to pick it up without needing to keep it on hand.

I am married with 3 kids. First two are ooop's while she was on the pill. I am pretty sure it falls under the missed doses heading.

If your female lacks general discipline, then she ain't taking that pill faithfully same time of day, daily. Mine isn't the best bill payer, not the best about cleaning up things that should be done daily around the house. I should have assumed she'd forget the occasional dose.

I'm not the guy who said it, but on Ali G some interviewee said that sperm can leak from the anus, cross the taint, and impregnate the girl. I don;t know if it's true, but it sounds feasible.
