Slow down your metabolism. All the fat and carbs and calories in the world will do you no good if your metabolism is speeding up fast enough to burn it off. This would go against doctor's orders, but start with eating large meals for breakfast/lunch/dinner and no snacking. High carbs, fat, and calories. Then knock out breakfast. Then make the meals smaller. Once you're damn near the point of eating like a 30 year old woman, start pounding back fast food like it's nobody's business.

Increase your portion size (although it sounds like you're trying this). With each meal try eating more than you usually do. Drinking a gallon of whole milk can help some people gain weight too.

You may want to calculate the caloric intake required to maintain your current weight and add more to see exactly how many calories you need to gain weight.

Also, snacking is good.

this. or if you want to be a jew about it, make protein shakes with oatmeal, milk, yogurt, fruit, and peanut butter. eat those like 2-3 times a day.
