Like I said, it's not worth the effort. If she's 300+ pounds something's seriously wrong with her diet. It's physiologically impossible to gain that kind of weight without stuffing your face with high carb meals frequently throughout the day. Sounds like the nurse is a problem too. If she can't/won't monitor her weight through simple nutrition and exercise, a person who's had a college education, what makes you think you can do it? What are her motives for getting you to do it? Because if she's with her 24 hours a day, this should be her job.
Are you even qualified in anyway to lead an exercise program? If not, for liability issues I wouldn't touch this with a 10 foot pole. Anything that goes wrong WILL be blamed on you. Sounds like the nurse's plan, or that she's given up. Either way, that tells you something about the situation. Its a FUBAR situation
