It also has to do with the fact that he didn't drop it, even though he knew he had to. Such an injury is unheard of in weightlifting, so it's a shame it had to happen at the olympics. Now every idiot newb with a childlike understanding of athletics is like, "durr it's unsafe ima do pilates". Weightlifting is extremely safe, with much less injuries compared to most sports.

what he did wrong was hold it too far back behind his head and didn't lock his elbows. Even though snatches are supposed to have the bar a little behind you, he overdid it and with a weight where you absolutely HAVE to use perfect form.

It really doesn't happen very often. 2 examples isn't enough to make it a common injury. Also, the total injury rate is much less than most other sports. When they're working with max weights and try to save it out of desperation, injuries like this can happen, but one of the first things you're taught is how and when to drop the weight. It's really their own fault.
