For the grapplers here, how much protective equipment do you use? I always wear a cup, and I'll usually use a mouth guard since the teeth always pay the price while trying to defend the RNC and it does help a bit for working cardio.

I think headgear is a bit excessive unless you have cauliflower ear, or are recovering from it.

headgear is really only necessary if you're competing in actual matches, during practices its just unnecessary because it has virtually no weight, so there's no added benefit of training with it on (unless you're trying to avoid cauliflower ear) I used to wear a mouthguard as well, its harder to get air in when you have to suck it through two small holes, but the amount of impact it takes for your jaw is a huge plus.

Depends on your gym.

I'd suggest doing it every now and then, so you are familiar with slightly reduced dexterity of your fingers. It's also a little harder to slide your hand into tight spaces and get wrist control.

I've never had a problem with mat burn. Even when it does happen I just suck it up and keep going.
