From what I understand, creatine does two things.
1: It increases the water retention of your muscles. This will give you larger looking muscles, but they will also be softer and more bloaty, as opposed to hard, cut muscle. This is one reason people may get cramps; if you don't drink enough water when using, and it gets taken up by your muscles, the rest of your body suffers.
2: It increases the rate of ATP production and recovery in your muscles. This means you have more endurance to pump out more reps, which means you can workout harder to improve the manner in which you grow muscle. It doesn't directly contribute to muscle growth, but will instead improve your workouts.
Also, some people do not experience either of these effects, and creatine is useless for them. For those who it works for, it's apparently quite a supplement.
