Kinda worried about my 18yrs old cat, he's lost his fur on a ~10cm spot below his neck (not shaven :|). What could this be?

When you say "below his neck" Do you mean "10cm below his jaw" or "10cm from the base of his neck, making the bald spot on his breastbone"? If the latter, its likely that he is grooming a baldspot on himself. Hell, if its the former, he's still possibly grooming himself. If there is something stressing him, it could be causing this, so quit pissing off the damn cat.

If the skin is red or crusty looking and the bald spot is circular, it could be ringworm. Get pics and I will diagnose it. Trust me, I am a registered Felifurologist.
