As for kittens dying.. I don't think it's neglect that gets them. Sometimes there is a kitten that just isn't as well developed. The kittens are neglected by the mother because she "knows" there is something wrong with them. I had this happen very recently.
Check the kittens and make sure they are eating, that they are warm, and provide the mother with good quality food and water nearby. If you can, give her some "pet milk" (KMR formula, there are other brands too), she'll love it and it'll help the babies. If a kitten starts to wane, try and bottle feed it or give it time alone with the mother (take the other kittens for awhile and give them a health check while the small kitten nurses). Sometimes though, they just die, and there isn't a lot you can do about it. It hurts a lot when you've spent all your energy taking care of them, though.
Two other things of use to newborn kitten "parents" :
Give kittens BenBac! It is a little tube of bacteria that helps regulate their digestive system. Incredibly useful if they get diarrhea. You can get it at most pet stores.
Sugar syrup! Sometimes a little bit of sugar can help a kitten by giving it enough energy to survive until you can really treat it. Put a spoon full of sugar in a bowl with just enough water to cover it, and microwave it until you can't see much of the sugar. Let it cool, then smear some in their mouths. It's not great to do that frequently, but can be a life savor. Examples of times when it can be useful: kitten gets too cold to digest milk, kitten falls or is injured in some way and you have to keep it alive until you get to the vet, etc. With sugar, a little goes a long way.
