My friend is having trouble with her cat.

He purposely went by the door and urinated,

Now he is old(14) but in great shape and never has bladder problems and is fixed.

Can anyone help by telling me why he did this?

Pic semi related it's not the cat I am talking about but another one of their cats.

well I can offer an interesting story that might help. I had a cat awhile back (male calico so an attitude is pretty much expected) and he used to pee on things when he did not get his way. One day he went as far as to pee on my little brother before school (I must say it was rather funny for me). Anyway we kept him outside for a couple months and he never did it again.

So maybe he is just doing it because of annoyance (the age of the cat makes me think otherwise though), but unless you have had a recent checkup with a vet, I would say have his bladder checked agian just to be sure.

Sounds like he's either scent marking, bored, or stressed.

The fact that he went by the door specifically (any other places since?) suggests to me that he either smelled a fresh "invading cat" smell brought in by someone through that door, or saw one by the door through a window, or something similar.

Google Feliway. It will help.

Cats can go senile.

Kidney problem is very likely because of the cats age, and that can cause cats to pee in unusual places. Take it to the vet.

Has your friend moved any furniture lately?
A friend of mine had a similar problem, he moved some chairs to another room and shortly after, his cat kept shitting and pissing in the house even though a litter box was accessible at all times.
A vet later told him that the cat might be literally pissed of by changes in the enviroment it´s used to and might use the shitting and pissing as an act of rebellion.
Kitty went back to normal when the chairs were moved back to their original position.

In OPs case it might be old age though.
