Blue on the left
His right side has been swollen for almost a week now
I've been giving him Maracyn for a few days
No decrease in appetite and he's moving around fine

What is wrong with him -- how do I know if it's a worm or something

worms or something wrong with the swim bladder. More likely that it's swim bladder if it's only on the right side. Also, look at his poop, if he has a trail of poop stuck to his ass or white stringy things coming out it's worms. But based on what you said and the picture I'd say it's more likely something wrong with his swim bladder. He may or may not recover and he may be deformed for life. Fish that get something wrong with their swim bladder in an aquarium are actually lucky, because if it happens in the wild it's a death sentence. He just may swim a little funny the rest of his life.

Most likely has a tumor
very common in older fish with saturated darker pigments
they are very suseptible to cancer becoz of linebred genetics
Can try to crush garlic, then squeeze pulp thru cloth to get juice to fall on dry food.
then feed soaked food to fish.
this smashes many random disease.
also possible to feed skinned frozen peas (yes I know is not norm a veggie eater) and ya will clear out blockages.
but most likely a tumor
