My sister has a yorkie terrier that is 4 years old. Anyways, his nails haven't been cut in a few months and one nail on the side of the foot, the dew claw has started to enter the padded part of the toe. My sister insists he is fine and he can wait a bit before it becomes urgent. I'm sure she is wrong, what would happen if not treated in a month?

You should definitely clip them. I had a Great Pyrenees that had it's nail nearly grow into the pad because he would nip at anyone who tried to cut it (at 140 lbs, by "nip" I mean "rend flesh from bone"). It doesn't get any easier; you just have to do it for the sake of the dog.

The nail will eventually grow into the pad, probably will become infected and will be extremely painful for the dog. Just go to your vet an get the nails trimmed, or you will have to pay huge bills to treat the infection caused by not cutting it.

I tried something once with my westie when he had that problem, and i know it sounds cruel. But I mixed some Malibu with some Ceaser wet food, and he went nuts for it. He was so lethargic after he didn't care that i clipped them. Now he lets me whenever I need to cuz he knows it feels better. And the little bastard may not be 140 lbs, but he might be able to clip a finger off.
